Macomb County’s Adult Day Health Service program provides the benefits of both Social and Medical models of care for individuals with physical and mental disabilities, allowing them to remain in a home setting while still receiving opportunities for socialization, supervision and medical care.
Individuals attend on a planned basis, which may range from one to five full or half days per week. The schedule is determined based on the participant’s and family’s needs.
The program also provides much needed respite and support for caregivers. Families are given a “break” and take comfort in knowing that loved ones are enjoying their day and receiving excellent health care and socialization.

Curbside transportation is available to Macomb County Residents within a 15 mile radius of the Center. Participants who elect to use program transportation are picked up at their homes in the morning and returned home at the end of the day by a lift-equipped vehicle provided by Macomb County Community Action Agency’s van/bus service.
